Dear Patient,
I am writing this letter to let you know that I have made the decision to retire from medical practice. I will be seeing my last patients in the office on 7/30/2026. I have truly loved my work and the privilege of caring for my patients. While it is not easy to step away from a profession that has been such an important part of my life, I have reached a point where I would like to devote more time to my family. I am so grateful for having had the opportunity to provide eye care for patients in Grand Island for the past 37 years. I appreciate the trust you have placed in me to care for your medical and surgical ophthalmic needs over the years.
It is important that you continue with appropriate eye care; therefore, I highly encourage you to contact your health insurance provider, primary care provider, or other trusted people in your lives to obtain the name of an eye care provider for your ongoing health needs.
Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only through your permission. Please let us know to whom you would like copies of your medical records forwarded.
I have greatly valued my years as a physician, and I would like to sincerely thank all of you who have trusted me to deliver the highest quality eye care. I will miss caring for you and I cherish the relationships we have built. I wish you good health and all the best in the coming years.
